Prior to enrollment you, the parent or guardian must meet the administrator to discuss and complete the following forms:

  • JFS 01634 Parent/Provider Agreement

  • JFS 01297 Child Enrollment and Health Information

  • A medical statement indicating that your child has been examined by a healthcare professional. This must be submitted within 30 days of the first day of care.

  • Transportation Form

Additionally, the following must be completed each year:

  • Updated medical statement

  • Verify and update the enrollment form

  • Have your child examined at least once every 13 months

Anytime there is a change in the above information, you should communicate it to the administrator immediately to assure that current information is alwasy on file.
